Hello doctor,
I am a 34-year-old male weighing 220 lbs, and my height is 6 feet. My bladder has felt full for about two days now. Some drops have come out, but I have been unable to go in a little bit. My nurse cousin told me that I would need a catheter, and at this point, they would tube me up. I can afford this, but please tell me if I have to go to the hospital.

Do you suffer from constipation?
Are you on any form of medication for anxiety or other mental health issues?
Do you use recreational drugs regularly?
Do you have any history of kidney stone disease?
If I am reading this correctly - this has happened thrice before, and you have been catheterized for it - what happened back then?
How long ago was this?
Did they scan you to diagnose the reason behind this (ultrasound/CT scan/MRI/ urodynamic study)?
Were you ever prescribed a medication- alpha-blocker after your last urinary retention?
If you currently have urinary retention, unfortunately, the best way is to get a catheter placed to avoid any ascending infection to the kidney. I would suggest you get that done immediately.

Hello doctor,
Thank you for the reply.
They have said the same every time, that I am too young for this. I do not take any daily medications or antidepressants. I got the magnet X-ray scan twice. I was treated with the same doctor for all three prior occurrences. I lost my mom recently. I do not seem to appreciate the reaffirming and validation. Do I use this response with a prescribing physician? However, I got a first opinion to pass on to somebody.

I still need the following information, which I asked for in my previous reply. Once again, it would be advisable that you get a catheter placed immediately. In addition to this, you will require a urodynamic evaluation to check for the function of the nerves of your bladder and any possible obstruction.

Thank you for the reply.
1. I am neither diabetic nor hypertensive.
2. I had surgery to remove adenoids and have had a few stitches before.
3. I suffered two car wrecks and was kicked by a horse once.
4. I suffer from constipation only when I eat cheesecake.
5. I have not used recreational drugs since COVID lockdown.
6. I do not have any history of kidney stone disease.
7. I was prescribed an alpha-blocker once.
Once again, it would be advisable that you get a catheter placed immediately. Do I need a recommendation or prescription for it? Will they just give it to you? My work wanted a note saying it was doctor-recommended for some claims department.

I do not know if they would consider this consultation as a prescription for you to get a catheter placed, but you could try. The recommendation would be to get to the nearest hospital, get a catheter placed, see a urologist, and set up an appointment to undergo a urodynamic evaluation.
This will tell you the current state of your bladder, whether it is working adequately or not. Further treatment will depend upon the results of this test. You may have some kind of bladder outlet obstruction, which is aggravated since you are not on the alpha-blocker medication.
