Hello doctor,

My leg wound is taking time to heal. I do not have diabetes and underwent tests and taking medicines. I am slightly hypertensive. I hit a sharp table by mistake two months ago, which is how I received the wound. I was not bothered about the injury initially but gradually developed pain the next month. I consulted a local doctor and took medicine, but the wound did not heal. Therefore, I underwent venous Doppler, pus culture, and blood tests. I am taking Protol xl 50, Lipaglyn, Stamina, Dexorange, Storvas, Clopilet, and Trental. I took Cepodem for 20 days due to the same complaint. Please help.

  4. Tablet Zifi (Cefixime) 200 mg twice daily after food for five days.

  5. Tablet Emanzen D (Diclofenac and Serratiopeptidase) once a day for pain for up to three days.

Consult a specialist doctor, talk to them, and then take the medication.

I suggest the following measures:

  1. Avoid injury or trauma to the lesion.

  2. Avoid occlusion.

  3. Keep the area clean and dry.

  4. Keep checking on sugar levels.

Can I cover the wound after applying Mupirocin when going out? Should I leave it open? I am taking other medicines for BP (blood pressure), like Clopidogrel.

Do not leave the wound open. The most crucial step is compression wound by a firm elastic bandage. Regular changes of dressings after cleaning are needed and keep a check on sugar levels. Avoid chronic pressure or friction on the wound and any injury to the lesion. For seven days, oral antibiotics such as Augmentin (Amoxycillin and Clavulanic acid) 625 mg twice day after meals can be administered. Clean the area with Povidone iodine solution and normal saline with cotton gauze for two months. After proper debridement, apply Mupirocin or Framycetin cream at night for two weeks. Consult a specialist doctor, talk to them, and then take the medication. Decontaminate hands and take necessary anti-septic precautions.

According to the reports attached continue with Zifi (Cefixime) twice daily after food for seven days. Yes, you can continue Trental according to the dose advised by doctor. But do take care of present ulcer due to trauma or injury. Is there any history of varicosity, varicose veins, or swollen legs.

Varicose veins are there and a Doppler scan is done. I attached a copy of Doppler report. I have no fever and slightly swollen legs.

There is a thrombosis seen in Doppler, so continue taking Trental. There is no evidence of deep vein thrombosis. For traumatic ulcers, avoid further injury to the wound. Wound care and proper debridement are needed for one month. Apply two percent Mupirocin (antibiotic) cream twice daily in the morning and evening after cleaning of wound with normal saline and cotton gauze piece for two months. Regular changes of dressings are needed. Avoid pressure on the wound. Reduce salt intake in your diet. Avoid stress, as stress is a big contributing factor. Stay relaxed. Avoid excess sugar intake in your diet.

I have been taking Zifi 200 mg as per your advice, but now since the culture results are different from the previous one and I read Cefixime may not be effective on pseudomonas, I had to write to you for further advice. What to do and should I change the antibiotic?
