Are there medications that can reduce skin thickening?

Hello doctor,

I have thick skin on the finger joints and in the outer part of the fingers. It has developed over the past two years. I am currently treating it with creams only and no medication. However, itching is persistent.

Based on your descriptions and the provided picture (attachment removed to protect the patient’s identity), it seems you have lichen simplex chronicus (a skin condition caused by repetitive scratching or rubbing). It arises because of repetitive scratching or rubbing, which results in a thickening of the skin with variable flakes.

Lichen simplex chronicus usually describes a stable, itchy, thickened area or more areas; however, thickening of the skin occurs in any location that the patient can reach, like the nape of the neck, extensor forearms and elbows, scrotum, upper medial thighs, knees, lower legs, and ankles. In your case, it involves extensor aspects of the hand and knuckles.

Age-related dryness, venous insufficiencies like tortuous veins, weakness of blood vessel walls due to aging, and eczema in old age due to baths with hot water (asteatotic eczema) are common contributing factors.

Apply a moisturizing cream (moisturizer), wait 15 minutes, then apply topical steroids such as Propysalic NF lotion (Clobetasol and Salicylic acid). An occlusive dressing will be more helpful.

After applying Propysalic lotion, cover the area with a flat polythene sheet, followed by a cotton bandage. Leave the dressing for the whole night for two to three days. Occlusion is not needed in the daytime. It should be applied twice daily for two to four weeks. Also, take one tablet of Levocetirizine (an antihistamine) once daily for a similar duration.
