How risky are lung nodules for cancer in a tobacco user?

Hi doctor,

I am a 62-year-old man, a lifelong tobacco user, chewing tobacco, not cigarettes. Just got a CT back showing multiple small nodules in my right lung, at least three of them near the outer edges. The biggest one is about 9.3 mm x 6.8 mm, sitting at the top of the right lower lobe. How worried should I be about cancer?

I understand your concern.

With your tobacco history and those nodules, you need a CT (computerized tomography)-guided biopsy. It could be a few different things: TB (tuberculosis), lung cancer, sarcoidosis, and there is no way to sort that out without tissue. So, get that biopsy scheduled.

Possible causes include:

  1. Infections such as tuberculosis, especially if there is a cough lasting more than three weeks, fever, night sweats, weight loss, chest pain, or coughing up sputum or blood.

  2. Inflammatory conditions such as sarcoidosis may cause dry cough, shortness of breath, fatigue, swollen lymph nodes, skin rashes, joint pain, eye irritation, or unexplained weight loss.

  3. Lung cancer symptoms that raise concern include persistent cough, coughing blood, unexplained weight loss, chest pain, breathlessness, wheezing, or repeated chest infections.

The next step is evaluation by a pulmonologist, who may advise:

  • Repeat CT scan after a short interval to assess growth.

  • PET (positron emission tomography) scan if indicated.

  • Sputum tests are performed if infection is suspected.

  • Bronchoscopy or CT-guided biopsy if the nodule's appearance is suspicious.

A CT-guided biopsy is often considered when imaging suggests a higher risk, but whether it is done immediately depends on the exact shape, borders, density, and whether there are enlarged lymph nodes.
