Hi doctor,

I have a prominent and twisted vein, most likely the dorsal penile vein. It appears excessively bulged and visibly pronounced on the upper side of my penis. This vein becomes even more exaggerated and prominent when my penis is erect, and I can also feel it when I pull back the foreskin. I have concerns about the unusual bulging of this vein, which I do not believe is normal. It has been there for a long time, and I do not have pain or discomfort while urinating or ejaculating. Please assess this and provide guidance or reassurance.

I have reviewed the images you have provided (attachments removed to protect patient identity) and it looks like lymphangiosclerosis. This is the thickening of a lymph vessel around your penis. The reason for this is the blockage of the channels that carry lymph away from the penis. In most cases, this goes away on its own with time. However, it may not be so in some patients. It is a harmless condition, which will not affect your sexual life. If it continues to grow in size or start causing pain, then you may have to consider removing it surgically. However, I do not think it will be required. I suggest you do a penile doppler test to confirm it.
