Hi doctor,

My three-year-old child had a fever of 101.7 degrees Fahrenheit late at night, and I gave him 3 ml of Meftal P. The height is 3 feet, and the weight is 34 pounds. After four hours, the fever remained the same, and he vomited once in the early morning. Should I go to the emergency room?

  1. Apart from fever, does he have any other problems like cough, cold, and more?
  2. Is he fully alert, well-oriented, and speaking normally?
  3. Is he urinating normally?
  4. Is there any difficulty in breathing?
  5. Is he complaining of abdominal pain?
  6. Did he pass normal stool today?
  7. Is there any other family member with a similar problem?

If he is otherwise normal, vomiting may be due to fever or Meftal P (Mefenamic acid). It could also be a part of a viral infection or the onset of a stomach infection, which may lead to loose motions.

For fever, the best course of action is to administer this medicine:

  1. Take syrup Paracetamol (250 mg per 5 ml) - Give 0.135 fluid ounces as needed for fever. You can repeat it every four hours if necessary.
  2. Use Meftal P if the fever does not subside after one hour of giving Paracetamol. You can repeat Meftal P every eight hours if necessary.

No other symptoms; fever has reduced a bit. He vomited bile just now.

I apologize for the late reply as I am not at home and received your follow-up query after midnight. Since there are no other symptoms, and he is active and alert, bile may come if he vomits on an empty stomach. Here are my suggestions:

  1. Give him bland foods such as milk, rice, bananas, and more.
  2. For vomiting, give him this medicine: Syrup Zofran (Ondansetron) - 0.0676 fluid ounces once.
  3. Wait for about 20 minutes, then give a small amount of liquid. If there is no vomiting, then give solid food.
  4. You can repeat this every eight hours if vomiting persists.
  5. Keep an eye on his alertness, oral intake, and bowel movements.

If vomiting persists despite this medicine, then you need to take him to the hospital. Otherwise, viral fever may take some time to subside and does not require any other treatment.
