Why do I have severe menstrual cramps and abdominal pain for many years?

Hi doctor,

I am a 28-year-old with a height of 5 feet 2 inches and a weight of 50 kg. I am newly married. Since I was 14 years old, I have been bearing menstrual cramps, which result in very severe lower abdominal pain.

I understand your concern.

Let me start by saying that this is quite normal for this age. During the initial few years, the periods are irregular or even regular, and the uterus is still immature for seven days, just developing into healthy adult-sized uterus.

So every month, when you get periods, you may have a few blood clots and thick fleshy masses sometimes. This is more than normal bleeding. This has to pass through the narrow cervical canal. This is very painful, as the cervical canal or mouth of the uterus (the lower end which opens into the vagina) is very firm and will not yield.

So this causes excruciating pain till the main bulk of blood or clots is gone. You will be immediately relieved of pain after that clot goes, to get it again in the next periods when the heavy bleeding is about to start (usually within the first and second days only).

You can have relief by taking precautionary painkillers like Meftal-Spas (Dicyclomine and Mefenamic acid) just before the pain. You will have a permanent solution only after the cervix is permanently dilated, which occurs after a normal vaginal delivery of the baby. Now, since you are newly married, you may also have pain during the end of sex also, or also during passing urine. They will go away on their own. You do not need any medications.
