Do hydrosalpinx cause right-sided abdominal pain?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I took a pregnancy test a few days back, and it turned positive. I went to the doctor for an abortion, so I took four pills of Mifepristone. I bled heavily with clots for two days, then it became normal; however, I have been facing immense pain in my abdomen on the right side.

  1. When I did sonography, it was also detected that I have hydrosalpinx in the right ovary.

  2. What should I do about the abdominal pain?

It feels like appendicitis, but I did not get any nausea, fever, or vomiting, just pain, and it pains more while urinating or common movements.

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

The right-sided abdominal pain you are experiencing after a medical abortion could be due to several possible causes, especially since you also have hydrosalpinx (fluid-filled fallopian tube).

Possible causes of your pain:

Incomplete abortion (retained tissue): Even though you had heavy bleeding, some pregnancy tissue might still be inside the uterus, causing cramping and persistent pain.

Symptoms:

  1. Ongoing moderate to severe pain (especially on one side).

  2. Light bleeding or spotting that continues for weeks.

  3. Possible infection risk if tissue remains.

Next step: Ultrasound to check if abortion is complete.

Hydrosalpinx-related pain (infected or inflamed fallopian tube): Since your ultrasound already detected hydrosalpinx in the right tube, this could be the main cause of your pain worsening with movement or urination.

    Possible complications:

    1. Infection (salpingitis).

    2. Fallopian tube torsion (twisting).

    3. Rupture (rare, but severe).

Next step: Gynecologist consultation + possible antibiotics or drainage if the infection is suspected

Pelvic infection (PID - pelvic inflammatory disease): An abortion increases infection risk, especially if bacteria enter the uterus.

      Symptoms:

      1. Severe pain, especially on the right side.

      2. Painful urination or sex.

      3. Fever (sometimes, but not always).

      4. Foul-smelling vaginal discharge.

Next step: Pelvic exam and antibiotics if infection is confirmed.

Appendicitis (less likely but serious): If pain is sharp, persistent, and worsening, appendicitis is a possibility. However, since you do not have nausea, vomiting, or fever, it is less likely.

Next step: If pain worsens, get an urgent ultrasound or CT (computed tomography) scan.

        What should you do now?

        1. Get an ultrasound as soon as possible – To check for retained pregnancy tissue, worsening hydrosalpinx, or infection.

        2. Consult a gynecologist immediately – If infection or complications are suspected, antibiotics or a procedure may be needed.

        3. Monitor symptoms – If pain gets worse, spreads, or fever develops, seek urgent medical care.

Would you like help understanding the best treatment options for hydrosalpinx based on your situation?

I hope I have answered your question.

Let me know if I can assist you further.

          Thank you.
