What does bleeding after my positive pregnancy test at 25 mean?

Hello doctor,

I am 25 years old. My last period is on the first day of the last month. I have done a pregnancy test that shows positive, and then my bleeding started on mid of this month same day, my sonography shows no g-sac (gestational sac, fluid-filled sac for embryo). Can you explain it?

I can see the details of your scanned report. You had a positive urine pregnancy test, then bleeding started, and an ultrasound in the middle of this month showed no gestational sac. With the last period on the first day of the previous month, by now something should have been visible if the pregnancy was continuing. The bleeding with no sac usually suggests either an early miscarriage or what is called a biochemical pregnancy.

I suggest doing the following tests;

  1. Serial beta-hCG (human chorionic gonadotropin) levels (repeat every 48 hrs) to see the trend.

  1. Repeat ultrasound after 1–2 weeks if beta-hCG remains positive.

  1. Basic blood work (hemogram) if bleeding is heavy.

If bleeding is not heavy, usually no emergency treatment is needed. However, if excessive bleeding, severe pain, or dizziness is present, go to the hospital immediately.

In future cycles, plan an early pregnancy check-up as soon as the urine test turns positive. Also, you can follow the following things to avoid miscarriage;

  1. Start taking folic acid before conception.

  1. Maintain a healthy lifestyle, avoid smoking or alcohol.
