Hello doctor,

My nephew who is 30 years old has been struggling with multiple sclerosis and has been experiencing unusual speech difficulties. He is a high school teacher, and sometimes he suddenly starts stammering mid-sentence or completely loses his train of thought. During class, he occasionally forgets words or mispronounces them, which is embarrassing for him. His students have started noticing these changes, and he is worried about his professional reputation. Some days, his speech is normal, but on other days, it feels like his brain and mouth are not communicating properly. Are these issues typical symptoms of MS? Is there anything that can help him manage these unpredictable speech challenges?

Multiple sclerosis (MS) is a condition that affects the brain and spinal cord. While it cannot currently be cured, treatment can often help manage it. There are many possible symptoms of MS, and everyone with the condition is affected differently. Some of the most common symptoms include:

  1. Feeling extremely tired (fatigue).
  2. Problems with your eyes or vision, such as blurred vision or eye pain.
  3. Numbness or a tingling sensation in different parts of the body.
  4. Feeling off-balance, dizzy, or clumsy (uncoordinated).
  5. Muscle cramps, spasms, and stiffness.
  6. Problems with memory and concentration.
  7. Erectile dysfunction.

Symptoms may come and go (known as "flare-ups" or "relapses" and "remissions") and may worsen over time.

Tests to diagnose MS may include:

  1. Evaluations of movement, coordination, vision, balance, and reflexes.
  2. Blood tests.
  3. MRI (magnetic resonance imaging) scans to detect nerve damage in the brain or spinal cord.
  4. Lumbar puncture (spinal tap) to take a sample of spinal fluid from the lower back.
  5. Tests with small sensors attached to the skin to measure how quickly messages from the eyes or ears travel to the brain.

Treatments you may receive include:

  1. Steroid medication to reduce swelling and help nerves function better.
  2. Disease-modifying therapies (DMTs) to reduce the frequency and severity of relapses.
  3. Muscle relaxants to relieve muscle spasms, cramps, or stiffness.
  4. Medications to treat pain, vision problems, and other symptoms.

You will also be offered treatment and support tailored to your symptoms. Every person's needs are different. Some forms of support that might be provided include:

  1. Advice on managing fatigue.
  2. Physiotherapy and exercises to address movement problems and muscle pain.
  3. Mobility equipment (for example, walking sticks, frames, or wheelchairs) and home adaptations (for example, stair lifts or railings) to help with daily activities.
  4. Cognitive behavioral therapy (CBT) to address anxiety, depression, and fatigue.

  6. Advice, exercises, and medications for bowel or bladder problems.
