Why are my 2-year-old’s nails separating from the base?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My two-year-old’s fingernails and toenails are pulling away from the lunula. Can you explain what might cause this?

Kindly help.

Thank you.

Hi,

Welcome to icliniq.com.

Thank you for providing such a clear history. From what you describe, with nails peeling or separating from the base (near the lunula) after a recent illness with rashes, boils, and antibiotics, this is most likely a condition called onychomadesis.

Onychomadesis often happens in young children after infections, high fevers, or serious skin illnesses. The illness causes a temporary pause in nail growth at the root. After a few weeks, the affected part of the nail may loosen and fall off. Although this can look worrying, the good news is that it is usually harmless and goes away on its own. New, healthy nails usually grow back without lasting problems.

There is no specific treatment needed, but you can:

  • Keep the nails clean and trimmed.

  • Avoid pulling or forcibly removing loose nails.

  • Maintain good hygiene to prevent secondary infection.

Please seek medical review if you notice:

  • Pain, redness, swelling, or pus around the nails.

  • Your child is becoming unwell again.

  • No new nail growth over the next few weeks.

To sum up, this seems to be a temporary nail change after illness, and your child’s nails should grow back normally with time.

I hope this addresses your concern.

Please feel free to share your feedback, as your input helps us provide better care.

Thank you.
