Hello doctor,

I am 28 and was just diagnosed with narcolepsy last month, after years of doctors telling me I was simply tired from being a new mom.

I experience sudden sleep attacks during the day, where I cannot stay awake, no matter what I am doing. This even happened while I was breastfeeding my 8-month-old baby, which terrified me. My sleep study showed that I went into REM sleep within three minutes, which my doctor said confirms narcolepsy.

I was prescribed Modafinil 200 mg in the morning, but I am still breastfeeding and worried about its effects on my baby. I have also been experiencing strange episodes where my knees buckle when I laugh or get excited; my doctor identified this as cataplexy.

Since my husband works long hours, I am alone with the baby most days and fear something might happen during one of my sleep attacks.

I have tried caffeine, but it makes me jittery and gives me headaches. My narcolepsy symptoms seem to have worsened since pregnancy and delivery. Can hormonal changes from childbirth trigger narcolepsy symptoms? I really need a treatment that is safe while nursing, because I do not want to stop breastfeeding yet.

Please help.

Your symptoms, like daytime sleep attacks, rapid REM (rapid eye movement) onset, and cataplexy, are very typical of narcolepsy with cataplexy.

The fact that they developed or worsened after pregnancy is not unusual. Hormonal and immune changes after childbirth can sometimes unmask or trigger autoimmune conditions, and narcolepsy is thought to have an autoimmune basis.

Modafinil is not recommended during breastfeeding, as it passes into breast milk and safety data in infants are very limited. Using it while nursing could expose your baby to unknown risks.

Safer approaches while breastfeeding include non-drug strategies such as scheduled naps, maintaining good sleep hygiene, and avoiding driving or activities that could put you or your baby at risk.

Unfortunately, most effective narcolepsy medications, including Modafinil, stimulants, and Sodium oxybate, are not considered safe during lactation.

In some cases, doctors may cautiously consider short-acting stimulants, but this requires careful risk–benefit discussion. It is also important to arrange as much daytime support as possible from your partner, family, or other caregivers, particularly during the months you wish to continue breastfeeding.

Once you finish breastfeeding, more treatment options become available, such as Sodium oxybate, which is very effective for cataplexy and excessive sleepiness, as well as other stimulants. Until then, the focus should be on safety and building support systems.
