Hello doctor,

I have a 14-year history of CRSwNP (grade 1 polyps) treated with nasal steroid sprays or drops. I have become more reliant on the stronger steroid drops, I have to use these once per day now. The condition is non-allergic. Symptoms have been persistent sneezing or rhinitis (though nasal steroids mostly eliminate this), recent symptoms are intermittent lack of airflow on the left side (which is worse when more steroids are applied for some strange reason). However, the symptom that bothers me most is a discomfort or feeling of fullness in both nasal passages. But appears again the next day. I have been thinking and do not know what to do as I guess at the moment my quality of life is affected about 30 %. I feel relatively ok about my airflow, it is just the discomfort.

Looking at your scans (attachment removed to protect the patient’s identity) all I can say is there is not much polyposis in your nose that is blocking your airway, there is just maxillary sinus thickening. Looking at the scans I would say your diagnosis is more in favor of CRS without nasal polyposis. And the DNS is very minimal, turbinates are enlarged. Here we assess the airway by doing a nasal endoscopy. Has a DNE been done? Do you get frequent sneezing? Considering your scan I would not recommend surgery.

Hello doctor,

My main symptoms are sneezing, running, and congestion. However, based on tests it is a non-allergic condition. Therefore I am wondering if my condition might be partly non-allergic eosinophilic. Recently I have required stronger nasal steroids. Could they be causing the feelings of on-and-off fullness or discomfort in my nose? This is the main symptom that bothers me. Do you think laser turbinate surgery under local anesthetic would be safe?

Your main symptom of blockage sensation is on the left side, is it? But the DNS is largely toward the right. If the eosinophil counts are high you should be a good candidate for immunomodulatory. There is a condition called hyperventilation syndrome if you are suffering from that then the surgery may not help.
