This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I often experience swelling on the right side of my neck, just beneath my jaw. It might be a lymph node. I have had this issue for approximately two years, and I cannot sleep on my right side because pressure on that area causes discomfort.

Please guide.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

A swollen lymph node under the jaw that has been present for approximately two years following a throat infection treated with Penicillin may have several potential causes.

While chronic swelling of a lymph node for this duration is uncommon with simple infections, it can occur due to persistent infections, immune system conditions, or, less commonly, malignancies.

Possible causes include:

  1. Effects of a previous infection, such as a scarred or fibrotic lymph node from the throat infection.

  2. Chronic or latent infections, such as tuberculosis or cat scratch disease. Immune-related conditions, including rheumatoid arthritis, lupus, or other inflammatory disorders.

  3. Rarely, lymphoproliferative disorders or cancers, such as lymphoma or metastatic malignancies, may be the cause, although these typically result in progressive growth or systemic symptoms.

  4. Other possibilities include benign cysts or soft tissue masses.

Considering that the swelling has persisted for two years, a clinical evaluation by a healthcare provider is advised.

Imaging tests, such as an ultrasound or CT (computed tomography) scan, can help characterize the lymph node. Blood tests may be necessary to determine the cause of inflammation or infection.

A biopsy or fine-needle aspiration may be necessary if there is a suspicion of malignancy or an unclear diagnosis. Since the swelling causes discomfort and affects sleep, further assessment is recommended.

Continue taking current medications as prescribed until you have your evaluation. Avoid pressing or massaging the swollen area, as this may aggravate the symptoms.

While many swollen lymph nodes resolve within weeks, a persistent swollen lymph node for two years requires further diagnostic workup to exclude chronic infections, immune-related causes, or rare malignancies, and to determine the appropriate treatment.

You can take the following tablets:

  1. Tablet Paracetamol 650 mg twice daily.

  2. Take a capsule multivitamin once daily.

I hope this information is helpful.

Please let me know if you have any further questions, and I would be happy to assist you.

Thank you.
