Hello doctor,
My 2-year-old son has been suffering from severe diarrhea and vomiting for the past three days. Recently, there was a norovirus outbreak in our locality, so we are concerned it might be related. We have been giving him ORS and Amoxyclav as advised by our doctor, but we are still worried. Is this likely to be due to norovirus, and is there anything else we should be doing to help him recover? Also, my wife is three months pregnant. What precautions should she take to avoid getting infected and protect the baby?

Norovirus is a highly contagious, self limiting infection. It can spread to all the members of a household with a single source of infection.Dehydration could be serious if water intake is not maintained.
Signs of severe dehydration
Lethargy or less activity than usual.
Pale and sunken eyes.
Cold hands or feet.
Faster breathing and heart rate.
Irritability, drowsiness, or confusion.
Dark yellow pee or less frequent urination.
Reduced skin elasticity.
If any of these signs are seen, please go to the ER (emergency room).
Norovirus infection is caused by a virus and hence Amoxyclav being an antibiotic will not help. Being a self limiting infection, only symptomatic treatment as mentioned below is required.
Also, Norovirus does not cross the placenta, hence it cannot directly affect the fetus. But it can cause dehydration which can affect the fetus. Hence maintaining hydration is very important if affected by norovirus infection.
Advice
ORS (oral rehydration solution), increased fluid intake, and avoid sugary drinks.
Antiemetics like Ondansetron for vomiting.
Anti-spasmodic like Cyclopam only after consulting a gynecologist.
Eat easy-to-digest food items like rice, soups, toast, and fruits.
Yogurt to maintain normal gut flora.
Preventive measures:
Maintaining adequate hygiene like hand washing, properly cooking the meat and vegetables, and cleaning the food properly before cooking. Disinfecting areas contaminated with norovirus is crucial due to the virus's highly contagious nature. It can be done by following the steps
1. Clean the surface first: Always use disposable gloves to protect yourself and others from exposure. Clean the area: Before disinfecting, clean the surface with soap and water to remove visible dirt and debris.
2. Use a suitable disinfectant: You can use a diluted bleach solution (one-third cup of bleach in one gallon of water). Ensure the solution is fresh and the area is well-ventilated when applying.
3. Apply the disinfectant: Ensure the disinfectant sits on the surface for the required time (usually five to10 minutes) to effectively kill the virus.
4. Wipe down surfaces: After allowing the disinfectant to sit for the recommended time, wipe down the surface with clean paper towels or disposable cloths.
5. Launder contaminated fabrics including clothing, bedding, or towels. Wash any contaminated fabrics in hot water (at least 140°F or 60°C) with a good detergent. Add bleach if safe for the fabric.
6. Dispose of gloves and materials properly. After cleaning, dispose of all disposable materials in a sealed trash bag to prevent further contamination.
7. Wash your hands.
