Hello doctor,
I am a 37-year-old woman experiencing ongoing challenges with obesity despite consistent efforts such as dieting, daily walking, and reducing my sugar intake. My BMI is 34, and I have been feeling fatigued, bloated, and emotionally overwhelmed. I would like to better understand, in simple terms, why my body may not be responding as expected. Could factors like hormones, thyroid issues, or PCOS be contributing?
I am also wondering whether weight-loss medications or injections are safe for women who may want to become pregnant in the future. In addition, I am concerned about the risks of developing diabetes and knee problems. Would it be more appropriate for me to consult an endocrinologist or a bariatric specialist?

I would like to reassure you that you are not alone. Conditions such as obesity, diabetes, high blood pressure, thyroid disorders, and PCOS are often interconnected and are considered lifestyle-related disorders. One condition can appear first and gradually lead to others, a pattern commonly referred to as metabolic syndrome. Many of these conditions also tend to run in families.
PCOS (polycystic ovarian syndrome) can cause symptoms such as irregular or delayed menstrual cycles, acne, facial hair growth, and thinning of scalp hair. It may also lead to insulin resistance, which can increase the risk of developing diabetes. These hormonal and metabolic changes can sometimes affect fertility and conception.
To help us move toward an accurate diagnosis and appropriate treatment plan, I recommend the following tests:
Thyroid function test.
Fasting blood sugar.
HbA1c.
Liver function test (LFT).
Serum lipid profile.
Pelvic ultrasound (USG – Pelvis).
In addition, please continue following healthy lifestyle habits. Aim for a balanced diet with limited salt and oil, and avoid deep-fried, greasy foods, bread, and other carbohydrate-heavy items.
