I have menopausal symptoms at 60. Can Estradiol gel cause it?

Hello doctor,

I am 60 years old, and it has been eight years post menopause.

I recently had a bilateral salpingo-oophorectomy, and I still have my uterus. I take Estalis continuous HRT patches and have for over five years, which worked well.

Since surgery, I have had mild recurring menopause symptoms, feeling hot, and a low mood. I also got hives, and my doctor has just added Estradiol gel one pump to boost my estrogen.

I have been taking it for six days so far, and nothing has improved.

After your bilateral salpingo oophorectomy, it is not unusual for hormone needs to change, even if you were already on HRT (hormone replacement therapy), because your ovaries (which still produce small amounts of hormones after menopause) are no longer present.

The return of mild hot flushes, mood changes, and even skin issues like hives can be a sign that your body is adjusting to lower estrogen levels. Adding Estradiol gel is a reasonable step, but it can take a few weeks, sometimes four to six, for noticeable improvements, as hormone levels stabilize and your body responds.

Since you still have your uterus, you must continue with a progestogen (which is already included in Estalis) to protect the endometrium.

If symptoms persist after several weeks, your doctor will consider adjusting the gel dose, switching to a different HRT formulation, or reviewing other contributing factors such as thyroid function, stress, or allergies in relation to the hives.

For now, patience is key, but keep monitoring your symptoms and let your doctor know if they worsen or if the hives become more severe.
