Hello doctor,

I have found a spot on the top of my gum. I first noticed a round shape two weeks ago, and there was no pain present. Now the round shape is growing in size and spreading its area with a white line around it. After some time, I found a small cut at the end of the gum of the tooth. I have been taking Neurobion Forte (Thiamine) once a day since then. As per my previous consultation with a doctor, it is a major herpetiform ulcer. I was wondering if I can do any tests and what is your recommendation. As well I had myokymia (eyelid twitch) that lasted for two weeks. Is there any antibiotic I can take?

I read your query and understand your concern.

I would like to advise you not to worry. I will help as much as I can. On the outlook, since you do not have pain, it looks like a simple oral submucous fibrosis with a bit of lichen planus. Although both conditions are not to be worried about. Do you have a history of tobacco chewing or smoking? If present, please stop the habits. Whenever possible, do the following investigations:

  1. Hemoglobin.
  2. Peripheral smear.
  3. Vitamin D.
  4. Vitamin B12.
  5. Folic acid.

Get back to me with these reports. Since you do not have any symptoms, we will not start you off with any treatment. You can write back to me if you need any further information.

I have done some tests you recommended. I could do the peripheral smear. Please see attached the report. I have just done the HIV screening, as it was included in the package. Nothing related to this issue. Could you please prescribe as per the report?

As I said earlier, it is a minor vitamin deficiency that is treatable. There is a minor deficiency of vitamin D for which you can take a tablet of Sunbless (Cholecalciferol) 60K, one tablet every week for four weeks, and then one tablet every month for the next four months. Then folic acid is on the lower end, for which you can take tablet Hifolate (L-Methylfolate, Pyridoxine, and Mecobalamin) 1 tablet every day for a month and then twice a week for four weeks. I think this should take care of all the symptoms.
