Hello doctor,
I have had an issue of faint orgasms without much pleasure for the past eight to ten years, which has worsened since last year. I have no issues with erections. I have also noticed slight tingling sensation, without numbness. My testosterone level was around 345 ng/dL earlier, but it has increased now to 900 ng/dL. However, there is no difference in the sensitivity issue. I can feel the urine when I urinate but do not feel much when I ejaculate. I take tablet Clopidogrel 75 mg, tablet Ramipril, tablet Clonazepam and tablet Crestor 20 mg regularly. I have not taken any medications for the sensitivity issue.

Thank you for your query. Let me know if there are any urinary complaints. There can be a possibility of retrograde ejaculation, where the semen, instead of going forwards, goes back into the bladder. If your urine shows sperm after ejaculation, there is a possibility of retrograde ejaculation. The second possibility is weak muscle contractions around the urethra, which pushes the semen out. If erections are good and orgasm is reached, we should not worry unless you want to plan for a baby. Retrograde ejaculation otherwise does not harm the body. So I suggest you to recheck your testosterone level, lipid profile, and blood sugar level, take a urine routine test after ejaculation, and exercise your perineal muscles.

Thank you for your reply.
I have had tests done. There is a good sperm count. The problem is just the decreased sensations during orgasm that are getting worse. I can feel the build-up of orgasm before but not during ejaculation, which is frustrating.

I understand your concern. It is possible that, after age 40, the prostate gland begins to enlarge and causes some obstruction to the urinary tract, which could change your sensations. However, if semen is coming out and you are reaching orgasm, I think we should not worry too much about the situation. With age, certain changes are expected, like erections not being as rigid, duration reducing, semen volume reducing, etc. This may be part of that process.
