Hello doctor,

Got my DEXA scan results back. My T-score is -2.7 in my hip and -2.4 in my spine. I am reluctant, as I have read some worrying reports regarding them, but my GP is recommending bisphosphonates.

Are bisphosphonates the sole medicine that will help my bones at the age of 55, or should I look into other choices before deciding to take them?

With a score of -2.7, it is crucial to start oral drugs to avoid additional bone resorption and to increase bone mass. In this case, oral bisphosphonates are preferable, particularly if the DXA (dual-energy X-ray absorptiometry) score is less than -3.0 and there are no fractures.

This type of drug is generally safe, and adverse effects are infrequent, so you should not worry about it. The most common side effect is heartburn and reflux of the esophagus. This can be reduced by taking the drug first thing in the morning while sitting, at least 30 minutes before meals.

Rare side effects are also possible. Jaw necrosis has been recorded primarily in individuals on high doses of the medicine for cancer, rather than in patients on regular dosages for osteoporosis, during dental treatments, because I have never seen one in my career and because atypical fractures are usually only noticed in persons who have been on the medicine for more than 10 years continuously.

Rest assured, your doctor is giving you the greatest care, so you have no reason to fear. It is also important to continue your physical therapy program, especially with weight-bearing and aerobic exercises, which can assist in increasing bone mass and preventing further loss.
