Patient's Query
Hi doctor,
I am a 57-year-old female, postmenopausal. I fractured my wrist last year after a minor fall, and my DEXA scan shows a T-score of –2.8 in the spine and –2.5 at the hip. I was told this means osteoporosis.
I am confused about whether to start Bisphosphonates, injections, or hormone therapy. I worry about side effects like jaw problems, which I read about.
Please help.
Thank you.
Hi,
Welcome to icliniq.com.
I have gone through your query and understand your concern.
With a T-score of -2.8 and a prior fracture, calcium and vitamin D are important but not sufficient on their own. You now require medication to actively strengthen bone and reduce fracture risk.
Treatment options include:
Bisphosphonates (Alendronate) are first-line tablets or IV therapy, very effective; the risk of jaw problems is extremely rare at osteoporosis doses.
Injections are a good alternative if you have stomach issues or prefer not to take weekly tablets; best considered after discussion with your doctor.
Hormone therapy is less commonly used as first-line at age 57 due to risks, but may be considered if you also have significant menopausal symptoms.
Lifestyle measures such as weight-bearing exercise and fall prevention remain crucial for maintaining bone health, but they cannot reverse osteoporosis to a safe level on their own.
I hope this clarifies your concern.
Thank you.
