What could cause ovarian tenderness and size changes?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I had an ultrasound today that showed tenderness when pressure was applied to my right ovary, which also had limited mobility and measured 11 mL in volume. My left ovary measured 19 mL. I experience painful periods about twice a year and started taking Slinda a month ago. Please help.

Thanks.

Hi,

Welcome to icliniq.com.

I can understand your concern.

It sounds like you have been through a lot today, and I understand how uncomfortable and worrying it can be to have pain during an ultrasound and hear about differences in ovarian size. The tenderness and limited movement of your right ovary could simply reflect normal variation or mild ovarian congestion, but sometimes it may suggest a functional cyst, inflammation, or adhesions from past infections or endometriosis.

Your right ovary’s volume of 11 ml and the left being 19 ml are both within a range that can still be normal, depending on the phase of your cycle and hormonal changes. Since you recently started Slinda (Progestrogen), which is a progestin-only pill, your body may still be adjusting to the hormonal shift, and this can sometimes affect ovarian activity and period pain temporarily. It would be best to follow up with your doctor or gynecologist to review the ultrasound findings in context, monitor any pain or new symptoms, and decide whether further testing or a short-term follow-up scan is needed. If your pain becomes severe, you develop a fever, or experience nausea, vomiting, or bloating, seek medical care promptly. You are doing the right thing by staying attentive to your health and sharing these details with your doctor.

I hope this information will help you.

Thanks.
