How can a man manage chronic burning urination at age 35?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 35 years old and have been experiencing a problem of burning during urination since I was young. I have never been cured despite trying many medicines over the years. I feel itching in the urethra, a burning sensation while urinating and during ejaculation, premature ejaculation, and difficulty getting an erection again. I have tried to treat this problem for many years, but there has been no improvement.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

The symptoms you are describing, like burning during urination and ejaculation, itching in the urethra, and sexual difficulties, suggest a chronic problem that may involve persistent infection, inflammation, or irritation of the urinary or reproductive tract. Since this has been ongoing since childhood and has not improved with medications, it is essential to re-evaluate the exact cause rather than continuing empirical treatments.

Possible causes include chronic prostatitis or chronic pelvic pain syndrome, which can lead to burning sensations, discomfort in the genital area, and sexual issues such as premature ejaculation or weak erections. Other potential factors include unresolved urinary tract infections, urethral strictures (narrowing of the urethra), or sexually transmitted infections that may have been inadequately treated in the past.

The best next step is to consult a urologist for a complete evaluation. They can perform

  1. Urine tests.

  2. Semen analysis.

  3. Prostate examination.

  4. Ultrasound.

If infection is ruled out, treatment may include medications to relax the prostate and pelvic muscles, anti-inflammatory therapy, and sometimes low-dose antibiotics for chronic inflammation. For sexual symptoms, addressing the underlying inflammation often helps, but additional treatments such as pelvic floor therapy or counseling for performance anxiety may also be beneficial.

Avoid alcohol, caffeine, and spicy foods, as these can worsen urinary burning. Keep yourself well hydrated and maintain good genital hygiene. With the correct diagnosis and specialist care, even long-standing problems like yours can often be improved significantly.

I hope I have answered your question.
