Hello doctor,

My father aged 54 years had PCA infarction (stroke) two years back. He is suffering from homogenous (right) hemianopia. Is there any treatment for this? Please suggest me.

How has the hemianopia fared over these two years? Is it increasing or decreasing? PCA (posterior cerebral artery) is one of the arteries supplying the visual pathway and its lesions usually cause visual field defects. Most of the defects will improve over time as and when the blood supply to the affected part (occipital lobe) will improve, unless of course the main trunk of the PCA is involved, which is known to cause persistent defects. The things which your father still has intact are his central vision, using which he can still read and perform most daily activities. Also his ability to move his eyes will also be working fine. Overtime he will be able to adapt using these two to be able to overcome the hemianopia, that is to say that he will be able to move his eyes and head to make the objects in the hemianopic field come into view. Now, coming to treatments and their results, eye movement exercises or scanning training like I mentioned in just a while ago. Incorporation of prisms into his glasses to enable seeing objects in the hemianopic field while head and eyes are straight, it can be an option in case his neck has a problem and he has problems moving his eyes. Both these treatments are given by orthoptists or optometrists. Regarding their usefulness in hemianopia, I will say that in most studies, patients usually improve and their fields come back to normal soon, so they never got enough time to train the patients with using these methods.
