Hello doctor,

I am 26 years old, weighing 209 lbs, and 5.5 feet tall. I am struggling with a hormonal imbalance. I suffer from irregular periods, thyroid issues, and polycystic ovaries. My lab results of ovarian tumor markers and thyroid test results are as follows:

1. TSH is 4.03 (range: 0-4.20).

2. T4 free is 1.04 (range: 0.93-1.71).

3. Testosterone-free is 4.0 (range 0.0-4.2).

4. Anti-TPO is 9.95 (range 0.00-34.00).

5. DGEA-S04 is 281.5 (range 98.9-340.0).

6. Fasting blood glucose is 105 (range 65-100).

I am currently taking Eutrix 500 mg. What medications can I take?

I just read your query, and from what you have attached (attachments removed to preserve the patient's identity), you have PCOS (polycystic ovary syndrome)/ PMOS (polyendocrine metabolic ovarian syndrome). Your body mass index (BMI), based on the weight and height you have mentioned, is 72 lbs/sq feet, and that is under the obese category.

However, you can take care of your PCOS/PMOS, and your weight will drop. I suggest taking Dianette (Cyproterone and Ethinyl Estradiol) oral pills. It is an oral hormonal contraceptive, but it regularizes your periods and eventually reduces PCOS. Consult your specialist doctor, discuss with them, and take the medications with their consent.

Secondly, I suggest taking the tablet Metformin 500 mg thrice daily after meals. It will help with weight loss, insulin resistance, and eventually PCOS/PMOS. Hello doctor,

Thanks for the reply.

I apologize for not mentioning that I have been currently inserting Jaydess for over a year. It is an intrauterine device that releases only progesterone. As a result, my weight increased, and my period has completely disappeared. I have, once in a while, some black spotting. Should I take any thyroid medicine as well? Should I change hormonal treatment?

Your absent periods are due to this contraceptive device that you have inserted that releases progesterone. Well, then you do not need Dianette (Cyproterone and Ethinyl Estradiol) anymore. As I have mentioned, I suggest you go for Metformin.

Consult with your specialist doctor, discuss with them, and take the medicines after taking them. There is no need for thyroid medicine, and there is no need to change the hormonal therapy.
