Hello doctor,
I am 57 years old. I was okay with my sexual performance. Now I am having weak erections, early ejaculation, and sometimes cannot do the insertion. Please help with your suggestions.

I read your query. I can understand your concern about your symptoms. It can be due to the following reasons:
Stress due to work or family-related night duties. Plan for regular exercise and yoga
Depression.
Varicocele (enlargement of the veins that transport oxygen-depleted blood away from the testicle).
Fear of failure; having one episode of ED once may make you feel you will end up with ED.
Low self-esteem (chronic masturbation).
Homosexuality.
Religious constraints.
Smoking and alcohol consumption.
Long-term illness (cardiac disease, thyroid disease, kidney disease, or any other disease) with long-term medication.
Negative attitude towards your partner.
Hormonal issue: Thyroid issue, low testosterone.
Long-term drug usage.
Diabetes and hypertension
You need to check Doppler penis, USG pelvis, and scrotum, blood pressure, RBS, free T3 T4, testosterone, TSH.

For a temporary solution, you can take tablet Mildfil (Tadalafil) 5 milligrams twice a day and tablet Duralast (Dapoxetine) 30 milligrams, 30 minutes before sex for 30 days.
