Hi doctor,

I have a small pimple-like structure on my external anus side. It started discomforting me today. I guess it is piles. I took pictures of it and compared them with available pictures of piles online. It looks exactly like that. What should I do? Can you suggest any cream?

As per your queries, the answers are as follows-

Rubber band ligation is when the hemorrhoid is tied off at its base with rubber bands, cutting off the blood flow to hemorrhoid. This treatment is only for internal hemorrhoids. Internal hemorrhoids may be classified into four grades based on the Goligher system-

  1. Grade I hemorrhoids protrude into the anal canal but do not prolapse
  2. Grade II hemorrhoids prolapse with straining or defecation but reduce spontaneously.
  3. Grade III hemorrhoid prolapse usually requires manual reduction.
  4. Grade IV hemorrhoids are prolapsed and cannot be reduced and are thus at risk of strangulation and thrombosis.

Grade I, II, and III hemorrhoids are managed medically, whereas grade IV hemorrhoids require interventions. Patients with symptomatic internal hemorrhoids in whom conservative management has failed or want to go for a nonsurgical therapeutic modality are candidates for band ligation.

Contraindications include thrombosis, colitis, rectal prolapse, rectal malignancy, or bleeding disorder.

The rubber bands used for this procedure are inexpensive and easy to deploy it is a daycare procedure usually done with local anesthesia. It is a good procedure and should be tried initially. The success rate varies from about 70 to 80 %. It would help if you took a stool softener like lactulose daily and a good quality probiotic like VSL 3 capsule.

There will be no smoking, alcohol, or alcoholic beverages. Take food that is easy to digest and has fiber. Avoid meat.
