How can a 40-year-old woman manage ADPKD?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 40-year-old woman who was recently diagnosed with autosomal dominant polycystic kidney disease (ADPKD) after an ultrasound for back pain.

My father developed kidney failure in his 50s, and I am scared that I might face the same outcome. My blood pressure has been fluctuating, and at times I feel fullness and pain in my sides. The doctor mentioned a drug called Tolvaptan.

  1. How effective is it in slowing kidney damage, and what are its long-term side effects?

  2. Should my siblings also get screened?

I would like to know if dietary changes or avoiding certain medications could help protect my kidney function.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

I completely understand how unsettling it must feel to receive a diagnosis of autosomal dominant polycystic kidney disease (ADPKD), especially with your father's history of kidney failure. ADPKD is a hereditary condition characterized by the gradual development of multiple cysts in the kidneys. While it can eventually impact kidney function, early monitoring and treatment can significantly slow the progression.

Tolvaptan is currently the only medication proven to slow cyst growth and preserve kidney function in ADPKD. Studies show that it can reduce the rate of kidney enlargement and delay the decline in kidney function, especially when started early in the disease. However, it must be used under close medical supervision because it increases urine output and can cause dehydration and elevation of liver enzymes.

Regular blood tests are required to monitor liver health during treatment.

It would also be wise for your siblings to undergo screening with a kidney ultrasound, since the disease is inherited in a dominant pattern, meaning each child of an affected parent has a 50 percent chance of inheriting it.

Lifestyle measures can also help protect your kidneys.

  1. Maintain well-controlled blood pressure, and avoid NSAIDs (non-steroidal anti-inflammatory drugs) such as Ibuprofen.

  2. Stay hydrated.

  3. Limit sodium intake.

  4. Follow a balanced, low-sodium diet rich in fruits and vegetables.

  5. Caffeine intake should also be moderate, as high amounts may encourage cyst growth.

With careful management, regular follow-up, and healthy habits, many people with ADPKD maintain good kidney function for decades. You are not alone in this, and with today's advances in treatment and early monitoring, your outlook is far better than it was in previous generations.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
