Hello doctor,
I am 36 and have moderate to severe psoriasis covering about 40 percent of my body that is not responding adequately to topical treatments anymore.
The plaques are thick, painful, and constantly flaking, leaving skin debris on my clothes and furniture that is embarrassing in professional settings. The itching is so intense that I scratch until I bleed, especially at night when I am trying to sleep, and the open wounds sometimes get infected.
Topical steroids provide temporary relief but cause skin thinning, and the psoriasis returns worse than before when I stop using them. My dermatologist mentioned systemic treatments like Methotrexate or biologics, but I am scared about immune suppression and potential side effects, including liver damage.
The condition has spread to my scalp, making it impossible to hide with clothing, and people stare or avoid shaking my hand, assuming it is contagious. My self-confidence is destroyed and I have started declining social invitations and work events because I am embarrassed about my appearance. The joint pain that started recently has me worried about developing psoriatic arthritis on top of the skin problems.
