Hello doctor,

My four-year-old son has been having a high fever for three days now, and his breathing seems labored. He got the pneumococcal vaccine when he was little, but I am worried he might have caught pneumonia anyway.

His daycare teacher mentioned there has been a respiratory illness going around.

  1. Should I be concerned that the vaccine did not work properly?

  2. Also, what symptoms should I watch for?

  3. Would that mean I need to rush him to the emergency room immediately?

The fever keeps coming back even after giving him medicine, and I am getting really scared. I am also trying to conceive my second child; will this infection affect my fertility or ability to get pregnant?

Pneumococcal vaccine offers strong protection but does not cover all strains; children can still get pneumonia or other viral respiratory infections despite vaccination.

Labored breathing, persistent high fever for over three days, and recurrence despite fever medicine are signs that urgent medical evaluation is needed.

The emergency signs you must look for are rapid breathing, grunting, chest retractions (visible pulling in of chest muscles), bluish lips or face, unresponsiveness, or inability to drink or wake up. These signs require immediate ER (emergency room) care.

Since a respiratory illness is circulating in daycare, it could be viral like RSV (respiratory syncytial virus), influenza, or adenovirus, which may still cause severe symptoms in young children.

Keep him hydrated and monitor his urine output; dry diapers or dark urine can be a warning of dehydration. Use Paracetamol or Ibuprofen as per weight and doctor’s advice to manage fever, but repeated spiking fevers should prompt a doctor review.

Get a chest examination and possibly a chest X-ray. A pediatrician can determine if it is pneumonia or something else.

Regarding your fertility, most common viral infections do not affect your ability to conceive, unless you become severely ill or have high fevers, which can temporarily affect ovulation.

You do not need to panic about fertility unless you get sick yourself; avoid close exposure if possible, practice hand hygiene, and consider wearing a mask if you are around him a lot.
