How to manage persistent cough and high fever?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 42-year-old female, and I have been battling what I think is pneumonia for the past week. My cough is persistent and painful, and I have had a high fever that will not break. I am taking the antibiotics you prescribed, but I am not seeing much improvement. Should I be concerned? Are there any additional tests I should undergo to rule out other conditions? I am also experiencing shortness of breath, especially when lying down. Is this normal with pneumonia, or could it indicate a more serious problem?

I am worried about the impact on my lungs. How long does it typically take to fully recover, and are there any long-term effects I should be aware of? Lastly, are there any over-the-counter medications or home remedies you would recommend to help manage my symptoms? I am eager to get back to my normal routine, but I want to ensure I am recovering properly.

Please guide.

Hi,

Welcome to icliniq.com.

I can understand your concern.

Given your symptoms, it is important to address your concerns promptly. While it is common for pneumonia to cause significant discomfort and take time to improve, the fact that you are not seeing much improvement after a week on antibiotics is concerning. This could indicate that the bacteria causing your pneumonia might be resistant to the prescribed antibiotics, or it could suggest a different underlying issue, such as a viral infection, secondary bacterial infection, or even complications like a lung abscess or pleural effusion. The shortness of breath you are experiencing, especially when lying down, can be a sign of worsening pneumonia, but it might also indicate other serious conditions, such as heart problems or fluid buildup in the lungs (pulmonary edema).

Plan for a CT (computed tomography) chest to assess the extent of the infection and rule out other conditions. Recovery time from pneumonia can vary widely depending on the severity of the infection, your overall health, and how quickly you receive effective treatment. It can take anywhere from a few weeks to a couple of months to fully recover, especially if the pneumonia is severe. To manage your symptoms while recovering, over-the-counter medications like Acetaminophen can help reduce fever and alleviate pain. Staying well-hydrated, resting, and using a humidifier to keep the air moist can also soothe your throat and make breathing easier. Expectorants like Ambroxol syrup can help loosen mucus, making it easier to cough up, while cough suppressants might provide relief if your cough is particularly harsh, but use them with caution and only if advised by your doctor.

I hope this helps.

Thank you.
