Hello Doctor,

I am having difficulty getting an erection after my foreskin is pulled back. However, I get normal erections when the penis is uncircumcised, especially during the night and in the middle of the day.

Your laboratory test results, including testosterone levels, thyroid function, blood sugar, and urine tests, are mostly within the normal range, which is reassuring.

The reduced quality of erections, specifically after pulling back the foreskin, might be related to local hypersensitivity or discomfort caused by stretching, particularly if you have had a tight foreskin or have only recently begun retracting it more often. Using delay sprays without first having an erection may also cause local irritation or reduce sensation.

It is recommended that you avoid using delay sprays for now, especially before achieving an erection, as these are better used occasionally and not regularly. Pelvic floor strengthening exercises, such as Kegel exercises, can help improve both erection quality and control over early ejaculation.
