Hello doctor,
I am 21 years old and unmarried. My last menstrual period likely started between the 10th and 12th of September and typically lasts no more than five days. However, I began bleeding again on the 29th of September. This time, the flow has been very light from the beginning and has continued for 9 days so far, and it is still ongoing.
The bleeding is mostly brown in color, and I occasionally experience pain on the left side of my abdomen. Please help.

Recommended investigations:
To better understand the cause, the following tests are advised:
Pelvic ultrasound – To assess endometrial thickness and ovarian condition.
Thyroid profile (TSH (thyroid-stimulating hormone), T3(Triiodothyronine), T4(Thyroxine) – To rule out thyroid-related issues.
Complete blood count (CBC) – To check for anemia (explain).
Urine pregnancy test – Just as a precaution, even if you are not sexually active.
Differential diagnosis to consider:
Dysfunctional uterine bleeding (abnormal bleeding from the uterus that is not caused by pregnancy, fibroids, polyps, cancer, or other identifiable structural problems).
Polycystic ovarian changes(appearance of the ovaries on an ultrasound, where multiple small fluid-filled sacs (follicles or cysts).
Endometrial or cervical inflammation.
Less likely but possible: early hormonal disturbance or thyroid-related issue.
Likely diagnosis:
The most probable cause appears to be prolonged, irregular bleeding due to hormonal imbalance.
Suggested management plan:
If the bleeding remains light and you are not experiencing weakness or dizziness, it is reasonable to monitor for now.
If the bleeding increases, Tranexamic acid 500 mg twice daily may be used to reduce it, but please consult a gynecologist before starting any medication.
Stay well-hydrated and include iron-rich foods in your diet.
Once the test results are available, we can decide whether a short course of hormonal therapy (like a five-day progesterone regimen) is necessary.

Please share your ultrasound and blood test results when available.
