Hi doctor,
I have been desperate about these prurigo nodularis lesions covering my arms and legs for the past nine months. My dermatologist tried topical steroids, oral antihistamines, and even phototherapy, but nothing worked. I am a female with a history of eczema, but this is completely different. I have large raised nodules that bleed when I scratch at night. Sleep deprivation affected my work as an elementary teacher. I started Gabapentin last month, but there was minimal relief, and I felt foggy during the day. Are there any emerging treatments? Could food triggers be involved? I am mentally exhausted from constant itching and visibility of lesions, causing social anxiety.

I can understand your concern.
By your history and description, we need to revise the diagnosis. So, get IgE level and eosinophil count. Get a skin biopsy and ANA (antinuclear antibody) profile. Once we get a diagnosis, we can start specific treatment. You might need immunomodulator drugs.
