Will topical steroids effectively treat Prurigo nodularis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 65-year-old male, who enjoys fishing and woodworking. For the past year, I have had intensely itchy, raised bumps on my arms, back, and legs that I cannot stop scratching. My dermatologist diagnosed me with Prurigo nodularis and prescribed topical steroids, but I feel like the itching is getting worse, not better. I have also noticed that some of the nodules are scarring over. Are there other treatment options that might help? Could this condition be linked to nerve issues or another underlying disease?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

Common issues with Prurigo nodularis-

  1. Nerve damage.

  2. Chronic Kidney or liver issues.

  3. Autoimmune diseases.

  4. Skin diseases like eczema (dry and inflamed skin), psoriasis (itchy dry autoimmune skin condition).

It is a vicious cycle as scar tissue can form from constant scratching, and it goes on.

Alternative treatment options-

1. Antihistaminic drugs like Levocetrizine.

2. Oral steroids for short duration followed by topical steroids for maintenance.

3. Immunosuppressive drugs.

4. Phototherapy.

5. Biologic drugs.

These are some of the available treatment modalities for Prurigo nodularis.

A comprehensive approach is needed for your illness.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.
