Can premature ejaculation be managed effectively at 36?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 36-year-old male. My weight is 207.23 pounds, and my height is 171 centimeters. The size of my penis when erect is 6 inches.

I do not ejaculate during foreplay, but as soon as intercourse begins, I ejaculate within approximately one minute. This has been affecting my sexual performance and causing significant distress.

Please tell me,

What are the possible treatments or strategies to improve my ejaculatory control?

Kindly help.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

I am very sorry about your medical condition, and I am glad that you are paying attention to changes in your body system.

What you are describing is premature ejaculation (PE), and it is one of the most common sexual concerns in men. You are not alone, and it is definitely treatable. Let us take a professional look at the possible related causes. This condition could be due to hormonal imbalance, anxiety, infection, or overactive penile nerve sensitivity.

May I ask if you had frequent masturbation in the past, painful urination, or if you are currently taking any medications?

I will advise you to undergo the following tests and share the results:

  • Hormone profile test.

  • Urine culture.

Along with investigations, the following strategies may help improve your condition:

  1. Stop, start method: During intercourse or masturbation, when you feel close to climax, stop stimulation, let the urge fade, and then resume.

  2. Squeeze technique: Gently squeeze the penis (just below the head) until the urge reduces, and then resume.

  3. Pelvic floor training: Strengthening the Kegel muscles (the same ones that stop urine flow midstream) can improve ejaculatory control.

I also advise the following medications:

  • Finasteride: 5 alpha reductase inhibitor: Take one tablet orally once daily for one month.

  • Lidocaine spray: local anesthetic: Apply topically before sexual activity as needed.

For concerns regarding penile size, I recommend that you consult a qualified specialist rather than rely on massage therapies, as evidence for enlargement through massage is limited and may cause harm.

I hope this helps.

Thank you.
