Hello doctor,

My tooth got a hole at the center (up to down) and it was hurting since years, but later it stopped hurting. Few months before the gums started to form inside the hole. Now the gums are swollen, and recently when I was brushing my teeth, a hole has formed on the sides of that swollen gums and it did not hurt. I tried sucking the gums and it started to bleed. So now I do not know what to do. Please help.

Actually, there is a term for the condition you are explaining. It is a dental polyp that develops in case of chronic pulpitis. As you have mentioned, the hole was there for years, and the pulp was exposed to the external environment and microbes for a long time, it has become an extensive carious exposure of the pulp.

First of all, I would suggest you to get a dental appointment as early as possible. It is already too late, and kindly do not delay further. The dentist will ask you to get an X-ray done for the affected tooth. An X-ray will clearly tell how far the pulp has been infected or what percentage of pulp is still vital to be saved. Depending on the severity, you will have to go for either root canal treatment or extraction. Root canal treatment can be done if the tooth is still in a condition to be saved, followed by capping with tooth-colored capping material. If the infection is really severe and there is no chance of saving the teeth, you might have to go for extraction. Post-extraction and healing, you can either go for an artificial teeth prosthesis with an implant or other supporting aids.
