What is the best treatment for juvenile rheumatoid arthritis with asthma?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am worried about my 14-year-old son, who has been experiencing joint pain and swelling for the past few months. He has asthma and has been feeling stiff in his wrists, knees, and ankles, mostly in the mornings. His blood tests show an RF of 110 IU/mL, anti-CCP at 180 U/mL, and CRP at 20 mg/L. His X-rays appear normal, but an ultrasound shows some early signs of inflammation in his knees. He uses inhalers for asthma. Since his grandmother had rheumatoid arthritis, I am concerned he might have juvenile rheumatoid arthritis as well. He is taking Ibuprofen and has been referred to a pediatric rheumatologist, but I am worried about the long-term effects. What treatment options are best to help him and prevent his condition from worsening, especially with his asthma?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

According to your description, it could be juvenile rheumatoid arthritis (JRA).

Early intervention is crucial to prevent joint damage and manage inflammation. Disease-modifying antirheumatic drugs (DMARDs), such as Methotrexate, are commonly used to treat JRA, and biologics like TNF (tumor necrosis factor) inhibitors may also be considered if needed.

It is important to consult a pediatric rheumatologist to balance treatment with his asthma management, as some medications may impact lung function. Regular follow-ups and monitoring will ensure his condition is managed effectively, reducing the risk of long-term complications.

I hope this has helped you.

Kindly follow up if you have more doubts.

Thank you.
