Hello doctor,

I have been experiencing recurrent skin problems in the area near my buttocks and occasionally around my waist, along with a history of piles. There have been rashes between my scrotum and buttocks, and on rare occasions, around the waist area. I have been using Lobate gel as advised by a doctor whenever these skin problems occur. Additionally, I am observing a white fluidic discharge from my buttocks. Kindly help.

Based on the information and pictures provided,(attachment removed to protect the patient's identity) it appears that you may have developed tinea incognito or tinea cruris, which is a fungal infection that may have been partially suppressed by the use of a topical steroid like Lobate.

It is crucial to discontinue the use of Lobate immediately and start the appropriate treatment for fungal infections for at least 15 to 20 days, during which you may notice an apparent increase in symptoms, which is the result of the suppressed infection resurfacing.

Tinea cruris is a fungal infection of the groin area. Here are some general measures you can follow:

  1. Keep the affected area dry. Use Clotrimazole powder frequently to maintain dryness.

  2. Avoid scratching or itching. Change your undergarments regularly. Prevent excessive sweating and avoid wearing damp clothes.

  3. Refrain from applying any general or steroid creams such as Betnovate, Lobate, etc.

  4. Ensure that you continue the treatment for an extended period even after the symptoms subside, to eliminate the fungal infection.

  5. Consider checking for diabetes and thyroid issues, especially if the infection recurs despite proper treatment.

  6. For treatment, you can apply 2% Ketoconazole cream (KZ Cream) in combination with Fucidin cream twice daily, along with taking Terbinafine 250 mg orally for two months.

Please ensure a follow-up appointment after one month to monitor the progress of the treatment.
