Hello doctor,
I am a 27-year-old male who has suffered from small and sometimes painful bumps, mainly on the back of my scalp but also sometimes on my sides. These bumps can sometimes have small white heads. I have been put on antibiotics and tried some topical creams with no luck.

From what you are describing, this sounds like scalp folliculitis, which simply means inflammation and infection of the hair roots. Bacteria usually cause it, and one frustrating thing about it is that it can keep coming back if not treated properly.
I know it can be annoying and persistent, but the good news is that it is manageable. It needs the right approach and consistency.
Right now, I would not suggest Isotretinoin. It is generally reserved for more severe or stubborn cases, and the dose you are on (if you have already started it) is too low to make a meaningful difference. We usually try a focused antibiotic and topical plan first.
As for the treatment plan, you can take
Doxycycline 100 mg once daily to calm the infection and inflammation from within.
Clindamycin lotion is applied twice daily to the affected areas.
Ciclopirox shampoo twice a week to keep the scalp environment controlled.
You will likely need this for about three weeks, and then we will reassess. Even after it improves, some people need occasional maintenance treatment to prevent flare-ups, which is quite common with this condition.
Try not to scratch or pick at the bumps, and avoid heavy oiling of the scalp for now. If you notice increasing pain, spreading redness, swelling, or fever, that would need quicker evaluation.
With steady treatment, this usually settles well. The key is sticking with the plan consistently rather than stopping too early once it starts to look better.
