Hi doctor,

I would like a second opinion about a revision rhinoplasty to correct a crooked dorsum (fixed), and a crooked columella (fixed). The bulbous tip was not fixed and is wider, and looks bigger. This is due to the nose being thin and the cartilage on the right side of the nose showing, making the lower part of the nose heavy on a thin columella. I have a petite nose that is out of balance with my features. Would a graft from a rib or ear be needed to fix this problem on the right side, making the tip appear bulky and spilling over? I have undergone a septoplasty for a deviated septum. Please guide.

The nose is bulky at the tip, but you can get it corrected by trimming the alar cartilage and removing excess subcutaneous tissues in it. Lower rib cartilage and Conchal cartilage in the ear can be used whenever in a shortage of nasal cartilage. They also give the best and same results as nose cartilages. But before going for revision rhinoplasty, I suggest discussing with your doctor about your ideas, about nose look so that there will be an outcome which will satisfy you.
