What could cause toothache after a root canal treatment?

Hello doctor,

Today the first root canal treatment session was over, but my toothache is not decreasing. During the treatment, the dentist said that my canals were extremely narrow, and at the last moment, he realized that there was one more canal and cleaned it. There are four canals in total, and he stated that there is inflammation in the root and that there may be pain. I had root canal treatment on my two molars a few years ago; I did not experience such pain. I am sure this is not normal. X-rays were taken. I use antibiotics and painkillers. What could be the cause of this pain? Kindly suggest.

I can understand your concern.

I am sorry for the pain you are going through. Sometimes, the pain during root canal treatment (RCT) goes till the second sitting, so there is nothing much to worry about, but make sure you are comfortable and pain-free during packing or the last sitting of RCT. Actually, the reason for pain can be any of the following:

  1. Canals not cleaned properly, which might take one more sitting.
  2. Inflammation or abscess around the tooth.
  3. The tooth is not healthy enough for RCT, but it is good that your dentist is trying to save it.
  4. Calcified canals which means there is very little or no space for root cleaning. In that case, you might have to go for extraction- but this can be confirmed only after looking at the X-ray.

Whatever the reason, your dentist will surely clarify whether there can be successful RCT after the second sitting. Till then, continue your antibiotics and analgesic medicines and maintain oral hygiene.
