Patient's Query
Hello doctor,
My son is 25 years old and is volunteering at a wildlife sanctuary in rural India. He developed a high fever five days ago. He now has a strange black eschar where something appears to have bitten him.
His lymph nodes are swollen to the size of golf balls, and he is experiencing severe muscle pain that makes him scream during our calls. The local clinic mentioned scrub typhus but did not explain how serious it could be. He is becoming more confused each day and is even mixed up about which country he was in yesterday. Should we arrange an emergency evacuation?
Kindly advise.
Hello
Welcome to icliniq.com
I have read the query and understand your concern.
I am very sorry to hear about your son's condition. The symptoms described, including a high fever, a black eschar, swollen lymph nodes, severe muscle pain, and confusion, are strongly suggestive of scrub typhus. This bacterial infection can become severe and requires urgent medical attention.
The presence of confusion is particularly concerning because it may indicate involvement of the brain, severe infection, or other life-threatening complications. Scrub typhus can lead to complications such as inflammation of the brain (encephalitis), inflammation of the lungs (pneumonitis), acute kidney injury, liver involvement, low blood pressure, and multiple organ failure if treatment is delayed.
Treatment should be started as soon as possible with appropriate antibiotics, such as Doxycycline or Azithromycin, depending on the individual's age, pregnancy status, allergies, and the treating physician's recommendation. Early treatment usually results in a rapid improvement, whereas delayed treatment increases the risk of serious complications.
Given the severity of the symptoms, especially the worsening confusion, your son should be taken to the nearest hospital immediately. If the local healthcare facility is unable to provide appropriate diagnostic testing, monitoring, or treatment, emergency evacuation to a hospital with higher-level medical care should be arranged as soon as possible, based on the advice of the treating healthcare team.
I hope this information is helpful. Please feel free to ask if you have any further questions.
Thank you.
