How to manage scrub typhus in pregnancy?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I need urgent help. I am 24 weeks pregnant and have been diagnosed with scrub typhus. I have a black bite mark around my belly, and a rash is spreading everywhere. My doctor stopped Doxycycline when she came to know about my pregnancy. The fever is 39 degrees Celsius (102.4 degrees Fahrenheit), and the baby's movements have diminished. Platelets decreased to 48000 per ml (milliliter), yet liver function was quite high. There might be DIC. I am also experiencing odd chest pain; is it okay to take Azithromycin? Kindly suggest.

Hello,

Welcome to icliniq.com.

I read your query and understand your concern. I suggest the below:

Fetal monitoring: Pregnant women with scrub typhus should be closely monitored for fetal well-being as the disease can increase the chances of preterm labor, low birth weight, and fetal distress.

Maternal monitoring: Pregnant women should be closely monitored for signs of complications such as respiratory distress, cardiac problems, or bleeding disorders.

Pregnant women with scrub typhus should receive care from a multidisciplinary team.

Early recognition and treatment of scrub typhus in pregnant women is essential in preventing complications and ensuring the best possible outcome. Keep taking Azithromycin which is safe to take in your condition and in the meantime consult a specialist.

I hope this helps.

Take care.
