I have scrub typhus – should I be concerned about my symptoms?

Hello doctor,

I was diagnosed with scrub typhus yesterday, and I am panicked. I got this black scab thing on my back and rashes everywhere. I probably got bitten two weeks ago. When I breathe, my chest hurts, and I have a fever of 39 degrees Celsius (101.8 degrees Fahrenheit). Blood tests reveal AST/ALT >1000 IU per liter, platelets 42000 cells per liter, and ferritin >10000 nanograms per milliliter. I have been coughing up blood since the morning, and my right-sided chest pain is worsening. A CT scan reveals fluid in the lungs. What is going on?

Scrub typhus is a bacterial infection transmitted to humans by the bite of a mite that carries bacteria. It responds effectively to treatment if begun within a week or two. To treat, use Doxycycline or Azithromycin tablets as antibiotics against bacteria. Supportive drugs include Paracetamol for fever and Levocetirizine for chills and rashes.

Scrub typhus is known to spread to almost all organs if left unattended or if treatment is initiated recently or delayed after one to two weeks. It affects blood (thrombocytopenia), lungs (pneumonitis, pleural effusion, and collection of fluid in pleura as in your case), liver (hepatitis, liver failure), kidney (kidney failure), stomach (ulcer, acid reflux), intestine (obstruction), brain (confusion, seizures), weakening of muscles and bones, and can cause multi-organ failure.

The good thing is that scrub typhus is a treatable condition. Given your condition, it is recommended that you be admitted to the intensive care unit and closely monitored.

Injection of Azithromycin can be started alongside breathing exercises, platelet transfusion to treat thrombocytopenia, regular monitoring of liver function tests, a full blood count, and proper cleaning of scab wound eschar.

Having a healthy diet, proper sleep, an immune-boosting diet, and a healthy lifestyle improves overall well-being. Ferritin levels return to normal when the liver recovers, as the liver is a completely regenerative organ. The primary goal is to eliminate scrub typhus bacteria from your body completely. It is a completely curable disease, but needs to be monitored regularly till the parameters return to normal.
