How to manage asthma and sinusitis effectively?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a history of asthma and chronic sinus issues. Lately, my asthma has been acting up badly, and my persistent sinus problems seem to be making it worse. I constantly feel congested, with a runny nose throughout the day and a dry cough that would not go away.

Breathing has become especially difficult at night, and I sometimes wake up gasping for air. While my inhalers provide some relief, they are not enough, and allergy medications leave me feeling excessively drowsy.

I have been on and off antibiotics for recurrent sinus infections, but they keep coming back. Weather changes, particularly humidity and cold, tend to worsen my symptoms. Despite using an air purifier, I have not noticed much improvement. I am looking for a more long-term solution, as I do not want to rely on medications indefinitely.

Could you advise on effective treatment options for both asthma and chronic sinus issues?

Additionally, do diet or lifestyle changes play a role in managing these conditions?

Kindly advise.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

Effectively managing asthma with bronchodilators and inhalers is crucial. Well-controlled asthma reduces complications and improves overall well-being. Could you share if you have any known allergies, such as pollen or dust? Have you ever undergone an allergen test? Asthma combined with allergic rhinitis can complicate both conditions. Recurrent episodes may lead to chronic sinusitis, which can be serious.

Acute exacerbations of asthma require prompt treatment with oral steroids, bronchodilators, and inhalers. It is important to maintain regular use of corticosteroid inhalers to prevent flare-ups. For acute episodes, antihistamines like Montelukast can be helpful.

Antihistamines like Montelukast with Levocetirizine can help manage sinusitis symptoms. Decongestants and nasal saline rinses provide relief. NSAIDs like Ibuprofen can be used for pain management. Antibiotics should only be taken in severe cases, such as sinusitis accompanied by fever, headaches, or persistent pain. In chronic or severe cases, steroids like Prednisolone may be prescribed to reduce inflammation.

Wearing a mask, especially during allergy season or in polluted areas, can help minimize exposure to triggers. It is important to change the mask regularly. Maintaining a healthy diet rich in green vegetables, fruits, and protein-rich foods supports immunity and overall health. Annual flu vaccination is recommended to reduce asthma, rhinitis, and sinusitis flare-ups. Consulting a physician can provide further guidance on preventive care.

It would be helpful to undergo allergen testing to identify specific triggers. Assessing immunoglobulin E (IgE) and immunoglobulin A (IgA) levels can also provide insight into your condition. Kindly share more details about any specific allergies you have experienced. Let me know if you have ever undergone an allergen test before.

I hope this information helps. For further queries, you can consult me at iCliniq.

Thank you.
