Hello doctor,

I am experiencing pain in my small intestine. I have been going through this for many months. I take medications like Zantac and Nexium. All the tests are negative, and I am in good health, with no other problems identified yet in the test. Please help.

I thoroughly read your query and understand your concern.

The infection you are suffering from is called gastroenteritis or stomach flu. In gastroenteritis, the mucosa of the intestine is severely inflamed, causing symptoms like loose stools, indigestion, and vomiting. Most of the time, it is viral and is self-limiting in nature. However, sometimes, it is bacterial or protozoal also. A stool test should be done to rule out the cause of infection. I suggest you the following tests:

1. Ultrasound abdomen to rule out inflammation of the abdomen.

2. Stool test for bacterial or protozoal infection.

3. Fecal calprotectin to rule out intestinal inflammation.

Most likely, stomach flu is viral and is self-limiting. However, antibiotic therapy can be given in case it is bacterial or protozoal. Treatment suggested:

  1. Start taking Fybogel (Isabgol husk, Nimbu stava, and Sarjika Kshar) or Metamucil (Psyllium husk). It increases the bulk of the stool and improves digestion.
  2. Start taking a good counter-probiotic daily. It will gradually improve digestion. They are rich in good bacteria which assists digestion.
  3. Take lots of fruits like bananas, apples, and peaches.
  4. Drink lots of fibrous juices.
  5. Take light and easily digestible food.

Antibiotic therapy could be given as per the reports (attachments are hidden to protect the patient's identity) of the investigation. Imodium 4 mg stat dose can be taken to stop loose stools. After that, it can be taken at 2 mg twice daily for three days. Dicyclomine can be taken for spasmodic pains along with Nexium 40 mg. A course of antibiotics can be considered after stool test results. A stool test will make it clear if it is due to a bug or E.coli.
