Hello doctor,

I was bitten by a brown recluse spider 12 weeks ago. The wound has healed, but my leg is now going numb down to my foot, and it almost feels like there is no blood flow. The bite was on my right leg or hip. I am also experiencing chronic headaches. I am a 49-year-old woman. I am supposed to be on Tranquipam, but I recently moved and have not seen a doctor yet.

Please help.

Numbness, especially at night, and the sensation of poor blood flow may point to nerve compression or even vascular issues.
  2. Vascular compromise: If the tissue around the bite heals with scar tissue or there is ongoing internal inflammation, blood vessels in the area could be restricted. This might explain the feeling of "no blood flow" in your leg and foot. A Doppler ultrasound or similar vascular study can help evaluate circulation.
  3. Chronic headaches and mood changes: These could result from post-infection inflammation or rare systemic effects of brown recluse venom. Medication withdrawal, like Tranquipam (which contains Diazepam), can cause withdrawal symptoms like irritability, tension headaches, and anxiety if stopped abruptly. Stress or sleep disruption may also be contributing factors. A systemic immune response or mild toxicity may also be triggered by the bite.

Recommendations:

  1. Neurological evaluation: A neurologist can assess the leg numbness. They may order an MRI (magnetic resonance imaging) to evaluate nerve damage or inflammation and conduct nerve conduction studies (NCV) or electromyography (EMG) testing.
  2. Vascular studies: Consider Doppler ultrasound to rule out circulatory issues.
  3. Blood tests: To rule out systemic inflammation, vitamin B12 deficiency, or other metabolic causes of your symptoms.
  4. Medication review: Do not restart Tranquipam on your own. A healthcare provider can help you resume it safely or transition to another medication as appropriate.
  5. Symptom diary: Track the timing and severity of your symptoms, such as numbness, headaches, mood changes, and sleep patterns, to help your healthcare provider with diagnosis and treatment planning.
