What could cause spotting after taking Medroxyprogesterone?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My period was expected soon, so I started taking Meprate 10 mg twice daily to delay it. I am still taking Meprate, but noticed some bleeding at night, although my period has not fully started.

Please advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

Here is a clear explanation of what may be happening and what to expect:

Medication taken:

  1. To delay your period, you are taking Meprate (Medroxyprogesterone acetate) 10 milligrams twice daily.
  2. Your period was expected around the time you started the medication.
  3. You are still currently taking Meprate. You have noticed spotting or light bleeding at night, but not a full period.

What this likely means:

  1. Spotting or breakthrough bleeding can occur while on Meprate, especially if the uterine lining had already begun thickening before you started the medication.
  2. Hormone levels are fluctuating. As long as you continue Meprate, your full period is typically delayed, though mild spotting may still occur. Once you stop Meprate, a withdrawal bleed (period) usually begins within two to seven days.

What you should do:

  1. If you still wish to delay your period, continue taking Meprate, but minor spotting may persist.
  2. If you no longer need to delay, stop Meprate; your period will likely start within a week.
  3. If bleeding becomes heavy or painful, stop the medication and consult your doctor.
  4. If no period occurs within seven to 10 days after stopping Meprate, consider evaluating for hormonal imbalance or pregnancy (if relevant).

When to seek medical help:

  1. Heavy or painful bleeding while still on Meprate.
  2. No period after 10 days of stopping the medication.
  3. New or worsening symptoms (such as severe cramps, fever, or dizziness).

You are likely experiencing breakthrough bleeding, common with Meprate and generally not a cause for concern. You can either continue Meprate if you still wish to delay your period, or stop it and expect your period within a few days.

I hope this helps you.

Thank you.
