Hi doctor,

I am 22 years old and yesterday, I noticed that my left testicle hangs horizontally instead of vertically. It is noticeably lower than the other one, and my scrotum appears larger and hangs much lower. A week ago, I experienced a stretching sensation on the left side of my penis, similar to the feeling when we stretch and crack neck muscles. Afterward, I felt the left testicular veins becoming thicker and experienced pain. Yesterday, while comparing the thickness of the left testicular vein with the right one, I felt a shock-like sensation, similar to a current passing between the testicles, resulting in significant pain. A few hours later, I felt something liquid in my lower scrotum on the left side, which I could feel with my hand. Concerned about testicular torsion, I promptly went to the hospital. The doctor suggested an ultrasound, and the result showed no torsion but indicated an infection in the left testicle. The report mentioned a reduction in echogenicity in the left epididymis, increased vascularity on Doppler, and a small amount of fluid collection in the left scrotal sac suggesting left epididymo-orchitis and a mild left hydrocele. They prescribed antibiotics and other medications, which I have been taking for two days with limited relief. Last night, I woke up and found my left testicle lying horizontally.
